A few different versions of Madame Hydra have appear in the comics. A version of her as Viper appeared in the film Wolverine and now a Madame Hydra is on Agents of SHIELD. She is played by Mallory Jansen in the show Agents of SHIELD. That version of the character is a bit different in that exists in an artificial world called the frame work and was an LMD in the real world.
At one point in the comics the being known as Hive took over her body and gave her a monstrous appearance. However, once Hive was defeated she returned to normal.
